  • I am not impaired and am fit to operate heavy machinery.
  • I acknowledge that I have the skills and training to operate the machinery.
  • I will use 3 points of contact when climbing in or out of machinery.
  • I acknowledge that I am responsible for my own safety and enter the working zone at my own risk.
